Notice of Availability of Form 1095-C

In compliance with the Affordable Care Act (ACA), St. Bernard Parish School Board is required to make available to full-time employees the Form 1095-C, which provides information about your offer of health insurance coverage for the previous year.  Form 1095-C is now available upon request. If you would like to receive a copy of your Form 1095-C, please contact the finance department using one of the following methods:
